Position Summary:
We are seeking a licensed Physical Therapist (PT) to provide skilled therapy services to patients in a home health setting. The Physical Therapist will evaluate patient functional status, develop and implement individualized treatment plans, monitor patient progress, and educate patients and caregivers to support safe recovery and successful discharge. Therapy services are delivered in the patient’s home in accordance with physician orders and established plans of care.
Key Responsibilities:
• Provide skilled physical therapy services in accordance with physician orders and approved plans of care
• Perform comprehensive patient evaluations and functional assessments
• Develop, implement, and revise individualized physical therapy treatment plans
• Treat patients to relieve pain, restore function, and maintain maximum physical performance
• Monitor patient responses to treatment and report changes in condition to physicians and care team members
• Educate patients and caregivers on therapy goals, exercises, safety techniques, and use of equipment
• Document evaluations, progress notes, and discharge summaries accurately and timely
• Participate in interdisciplinary case conferences to ensure coordinated patient care
• Assist with continuity of care and discharge planning as appropriate
• Supervise Physical Therapy Aides in accordance with regulatory requirements
• Participate in quality improvement initiatives, staff meetings, and in-service education programs
Required Qualifications:
• Active, unrestricted Physical Therapist (PT) license in the state of practice
• Graduation from a CAPTE-accredited Physical Therapy program or equivalent
• Successful completion of a state-approved Physical Therapy licensing examination
• Minimum 2 years of experience as a Physical Therapist preferred
• Strong clinical assessment, treatment planning, and documentation skills
• Ability to work independently in a home-based care environment
• Excellent communication, organization, and time-management skills
Pay Rate
$65.00 - $79.47/hour
