Position Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality nursing care in a Rehabilitation and Care Center setting. The RN will support patients recovering from illness, injury, surgery, or chronic medical conditions by providing clinical care, coordinating rehabilitation needs, and collaborating with interdisciplinary teams to promote recovery, safety, and improved quality of life.
Key Responsibilities:
• Perform comprehensive nursing assessments and monitor patient health status
• Develop, implement, and evaluate individualized nursing care plans
• Administer medications, treatments, and therapies as prescribed
• Monitor patient progress and communicate changes in condition to providers and care teams
• Provide wound care, pain management, and chronic disease management as needed
• Assist patients with rehabilitation goals, mobility, and activities of daily living (ADLs)
• Collaborate with physical therapists, occupational therapists, speech therapists, physicians, and other healthcare professionals
• Educate patients and families regarding care plans, medications, and recovery expectations
• Maintain accurate and timely documentation in electronic medical records (EMR)
• Ensure compliance with infection control, patient safety, and regulatory standards
• Support discharge planning and continuity of care
• Promote a safe, respectful, and patient-centered care environment
Required Qualifications:
• Active, unrestricted Registered Nurse (RN) license in the state of practice
• Associate’s or Bachelor’s degree in Nursing (BSN preferred)
• Minimum 1–2 years of recent nursing experience preferred
• Experience in rehabilitation, skilled nursing facility (SNF), long-term care, or acute care preferred
• Current BLS certification required
• Strong clinical assessment, critical thinking, and care coordination skills
• Excellent communication, organizational, and interpersonal abilities
• Ability to work effectively in a team-oriented healthcare environment
• Experience with electronic medical record (EMR) systems preferred
Preferred Qualifications:
• Experience caring for geriatric and medically complex patients preferred
• Rehabilitation or post-acute care nursing experience preferred
• Knowledge of restorative nursing programs and care planning preferred
• Commitment to providing compassionate, evidence-based patient care
Pay Rate
$57.42 - $62.30/hour
