Job Title: Licensed Practical Nurse (LPN) – Long-Term Care (LTC)
Position Summary:
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a Long-Term Care (LTC) facility. The LPN will work closely with RNs, physicians, and healthcare staff to ensure patient safety, comfort, and well-being while delivering patient-centered care.
Key Responsibilities:
- Provide direct patient care in accordance with individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and record patient vital signs, conditions, and progress
- Assist residents with activities of daily living (ADLs)
- Maintain accurate and timely medical documentation
- Observe and report changes in patient condition to the RN or physician
- Follow infection control and safety protocols
- Respond promptly to patient needs and call lights
- Support residents with mobility, hygiene, and nutrition
- Collaborate with interdisciplinary healthcare teams to ensure quality care
Pay Rate
$37.90 - $42.78/hour
