LPN – LTC (Licensed Practical Nurse – Long-Term Care) Job Description
A Licensed Practical Nurse (LPN) in a Long-Term Care (LTC) facility provides basic nursing care to residents, usually elderly or chronically ill patients, under the supervision of an RN (Registered Nurse) or physician.
Key Responsibilities

Administer medications and treatments as prescribed.

Monitor and record vital signs (blood pressure, pulse, temperature, respiration).

Assist residents with daily living activities such as bathing, dressing, and mobility.

Observe patients’ conditions and report changes to the RN or physician.

Maintain accurate patient records and documentation.

Provide wound care and basic medical procedures.

Assist with care planning for residents.

Supervise and support CNAs (Certified Nursing Assistants).

Ensure patient comfort, safety, and infection control standards are maintained.
Requirements
-
Valid LPN/LVN license in the state of practice.

BLS/CPR certification required.
-
1 year of LTC or clinical experience preferred.

Strong communication and patient care skills.
Pay Rate
$42.23 - $47.11/hour
