LPN – LTC (Licensed Practical Nurse – Long-Term Care) Job Description

A Licensed Practical Nurse (LPN) in a Long-Term Care (LTC) facility provides basic nursing care to residents, usually elderly or chronically ill patients, under the supervision of an RN (Registered Nurse) or physician.

Key Responsibilities

  • Administer medications and treatments as prescribed.

  • Monitor and record vital signs (blood pressure, pulse, temperature, respiration).

  • Assist residents with daily living activities such as bathing, dressing, and mobility.

  • Observe patients’ conditions and report changes to the RN or physician.

  • Maintain accurate patient records and documentation.

  • Provide wound care and basic medical procedures.

  • Assist with care planning for residents.

  • Supervise and support CNAs (Certified Nursing Assistants).

  • Ensure patient comfort, safety, and infection control standards are maintained.

Requirements

  • Valid LPN/LVN license in the state of practice.

  • BLS/CPR certification required.

  • 1 year of LTC or clinical experience preferred.

  • Strong communication and patient care skills.

Pay Rate
$42.23 - $47.11/hour
