Description:
8-hour shifts, 5 days per week.

Position Summary:

We are seeking a dedicated and compassionate Physical Therapist (PT) to provide rehabilitative therapy services to patients in a Skilled Nursing Facility (SNF) setting. The Physical Therapist will evaluate patient mobility and functional limitations, develop individualized treatment plans, and implement therapeutic interventions to improve strength, mobility, balance, and overall functional independence. The role focuses on restoring patient function, preventing disability, and supporting safe discharge planning.


Key Responsibilities:

• Perform comprehensive physical therapy evaluations and functional assessments
• Develop, implement, and update individualized treatment plans based on patient needs
• Provide therapeutic interventions to improve strength, mobility, balance, and coordination
• Assist patients in regaining functional independence in activities of daily living (ADLs)
• Utilize therapeutic exercises, manual therapy, and modalities as appropriate
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes
• Educate patients and caregivers on exercises, safety techniques, and mobility strategies
• Collaborate with physicians, nurses, occupational therapists, and interdisciplinary teams
• Participate in care planning meetings and discharge planning processes
• Maintain accurate and timely clinical documentation in accordance with regulatory standards
• Ensure compliance with SNF regulations, patient safety standards, and infection control protocols


Required Qualifications:

• Active, unrestricted Physical Therapist (PT) license in the state of practice
• Graduation from an accredited Physical Therapy program (DPT or equivalent)
• Minimum 1–2 years of experience in SNF, rehabilitation, or geriatric care preferred
• Current BLS certification preferred
• Strong knowledge of rehabilitation techniques and mobility training
• Experience working with geriatric and medically complex patient populations preferred
• Excellent communication, organization, and interpersonal skills
• Ability to work effectively in a team-oriented and fast-paced healthcare environment
• Commitment to delivering patient-centered, outcome-focused care

Pay Rate
$58.90 - $63.78/hour
