Professional Summary

Licensed Physical Therapist with 1 year of home health experience providing skilled therapy services to diverse patient populations in residential settings. Experienced in comprehensive patient assessment, cardiopulmonary/circulatory care, orthopedic and neurological rehabilitation, documentation, billing, and patient/family education. Proficient in developing individualized home exercise programs and implementing safety and injury prevention strategies to improve patient outcomes and independence.

Skilled in the use of assistive devices and rehabilitation equipment including gait belts, rolling walkers, standard walkers, sit-to-stand lifts, sliding boards, soft/hard collars, supplemental oxygen equipment, upper extremity prosthetics, and wheelchair assessments. Experienced in caring for med/surg, cardiac, post-surgical, COPD, CHF, pneumonia, CVA, Parkinson’s, trauma, trach, AKA/BKA, falls, weakness, respiratory, and ADL-dependent patients within Skilled Home Care environments serving Medicare, Medicare Advantage, and commercial insurance populations.

Knowledgeable in recognizing signs and symptoms of physical, sexual, and psychological abuse, medication management, and maintaining accurate clinical documentation using BayCare Forms, BayCare Scheduling, and Ace Timesheet systems. Able to independently manage 6–8 skilled home visits per day while maintaining high-quality patient care standards.

Certifications & Qualifications

  • State Licensed Physical Therapist
  • BLS Certified
  • Home Health experience required
  • OASIS training preferred
  • Valid driver’s license and reliable transportation for home visits
  • Flexible with self-scheduling and weekend rotation requirements
  • COVID vaccination/exemption compliant and flu policy compliant

Shift Preference: Day Shift (5x8 Hours) | 8:00 AM – 5:00 PM
