Shift: Day (5x8 Hours) — 8:00 AM to 5:00 PM

Required Skills:
Assessment, Cardiopulmonary/Circulatory, Documentation, Equipment and Devices (Gait belt, Rolling Walker, Sit to Stand Lift, Sliding Boards, Soft/Hard Collars, Standard Walker, Supplemental Oxygen, Upper Extremity Prosthetics, Wheelchair Assessment), Ortho, Neuro, Home Exercise Program, Medications, Patient & Family Education, Signs & Symptoms of physical/sexual/psychological abuse, Safety and Injury Prevention, Billing

Must have home health experience; OASIS training preferred

Certifications Required: BLS, State Licensed
Scrubs: Any solid color scrub

Patient Types:
Med/Surg, Cardiac, Ortho (hips/knees/shoulders), Post-surgical, COPD, CHF, Pneumonia, CVA, Falls, Weakness, Parkinson’s, Failure to Thrive, Respiratory patients, Neuro, Trachs, Trauma, AKA, BKA, ADLs

Care Setting: Skilled Home Care (Medicare, Medicare Advantage, Commercial insurance)

Daily Workload:
Home skilled visits — 6–8 per day
(Minimum 6 points/day at manager’s discretion; may increase by 1 point)

Schedule Notes:
1–2 weekends per contract

Equipment Provided:
Laptop, MiFi for internet access, Portable printers

Systems Used:
BayCare Forms, BayCare Scheduling, ACE Timesheet documentation system

Other Requirements

Minimum 1 year experience required
Valid driver’s license and car required for home visits
Must be COVID vaccinated or have exemption on file
Must have flu shot or declination; mask mandatory at all times

Pay Rate
$56.30 - $61.18/hour
