| Job Summary | Conducts health assessment interviews focused on wound, ostomy, and continence-related care. Performs focused physical assessments, develops plans of care based on diagnosis and individual client needs, and recommends medications and treatments as appropriate. |
| Job Responsibilities | • Develops, revises, and evaluates standards of care; assists in the selection of supplies, technology, and equipment. • Assesses and documents patient response to wound care, ostomy, continence, fistula, and complex wound treatments; evaluates the quality and delivery of care provided by clinical staff on the unit. • Works collaboratively with the healthcare team and makes referrals to other disciplines as indicated. • Teaches, counsels, and assists individuals and families in assuming responsibility for prevention of complications related to ostomies, wounds, or continence problems. • Develops, provides, and evaluates staff education programs including new employee orientation, in-services, and continuing education programs. • Routinely provides comprehensive assessments of patients at risk for developing pressure injuries and initiates preventive measures. • Escalates and communicates identified challenges, including ethical issues and conflict resolution concerns. • Collaborates routinely with the Assistant Director of Nursing for Wound Care to initiate and recommend best practices based on prudent assessment. • Participates in Unit/Department Quality Improvement and hospital-wide QI activities, including incidence/prevalence studies and nursing quality initiatives. • Provides culturally competent patient, family, and staff education consistent with cultural, ethnic, religious, and other belief systems. • Maintains the security and confidentiality of data and information in accordance with patient rights policies. • Achieves and maintains professional competency through engagement in activities consistent with clinical practice and the population served. • Identifies available community resources helpful to clients; refers and assists clients in utilizing such resources. • Manages assets related to rented equipment, such as specialty beds and negative pressure devices. |
| Required Skills & Experience | • Two (2) years of Med/Surg experience. • Wound and colostomy experience. |
Pay Rate
$89.90 - $94.78/hour
