Position Summary:
We are seeking an experienced Registered Nurse to provide skilled nursing care to patients in post-acute and long-term care settings. The RN will manage patient care, administer medications and treatments, monitor clinical conditions, coordinate services, and support patients' recovery, rehabilitation, and quality of life.
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized care plans.
- Monitor patient conditions, vital signs, laboratory results, and changes in health status.
- Administer medications, IV therapies, and prescribed treatments safely.
- Provide wound care, catheter care, and other skilled nursing interventions.
- Manage patients recovering from hospitalization, surgery, illness, or injury.
- Monitor chronic conditions and identify signs of clinical deterioration.
- Coordinate care with physicians, therapists, pharmacists, social workers, and other healthcare professionals.
- Support rehabilitation, functional improvement, and safe transitions of care.
- Educate patients and families regarding medications, treatments, disease management, and care plans.
- Participate in admissions, care conferences, transfers, and discharge planning.
- Maintain accurate and timely documentation in the EMR.
- Follow infection prevention, HIPAA, patient safety, and organizational policies.
- Promote patient dignity, comfort, independence, and quality of life.
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- ADN or BSN from an accredited nursing program.
- Current BLS certification required.
- Minimum 1 year of recent post-acute, long-term care, skilled nursing, or acute-care experience preferred.
- Strong knowledge of medication administration, patient assessment, wound care, and chronic disease management.
- Excellent clinical judgment, communication, organization, and time-management skills.
- Ability to manage multiple patients and changing clinical priorities.
- Experience with EMR systems preferred.
Preferred Qualifications:
- BSN preferred.
- Experience in skilled nursing, rehabilitation, transitional care, or long-term care.
- Experience caring for geriatric and medically complex patients.
- Wound care and IV therapy experience preferred.
- Strong knowledge of care planning and interdisciplinary coordination.
- Experience with post-hospital transitions and discharge planning.
- Commitment to compassionate, patient-centered care.
Pay Rate
$54.37 - $59.25/hour
