RN – Inpatient Care Coordinator
We are seeking a skilled Registered Nurse (RN) – Inpatient Care Coordinator to coordinate patient care, facilitate communication among healthcare teams, and support safe and efficient transitions throughout the inpatient stay.
Key Responsibilities:
- Assess patient needs and coordinate individualized plans of care.
- Collaborate with physicians, nurses, case managers, and multidisciplinary teams.
- Monitor patient progress and identify barriers to timely care and discharge.
- Coordinate consultations, services, referrals, and follow-up care.
- Educate patients and families regarding treatment plans and discharge instructions.
- Maintain accurate documentation and promote continuity and quality of care.
Qualifications:
- Graduate of an accredited nursing program.
- Current and active RN license as required by the state.
- Current BLS certification.
- Previous inpatient, care coordination, case management, or acute-care experience preferred.
- Strong clinical assessment, communication, organization, and care-coordination skills.
Pay Rate
$86,000.00 - $117,000.00/year
