RN Case Manager – Cardiology
We are seeking a skilled RN Case Manager – Cardiology to coordinate comprehensive care for cardiac patients and support effective treatment outcomes across the healthcare continuum.
Key Responsibilities:
- Assess patients’ clinical needs and develop individualized care plans.
- Coordinate care among cardiologists, specialists, patients, and healthcare providers.
- Monitor patient progress, symptoms, treatment plans, and outcomes.
- Educate patients and families on cardiac conditions, medications, and self-care.
- Coordinate referrals, follow-up appointments, and transitions of care.
- Maintain accurate clinical documentation and promote continuity of care.
Qualifications:
- Graduate of an accredited nursing program.
- Current and active RN license as required by the state.
- Current BLS certification.
- Previous cardiology, case management, or acute care nursing experience preferred.
- Strong clinical assessment, communication, and care-coordination skills.
Pay Rate
$86,000.00 - $117,000.00/year
