Job Description – Home Health RN

Shift:
Day Shift – 5x8 Hours
8:00 AM – 5:00 PM

Position Overview

We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled in-home patient care. The ideal candidate must have strong clinical experience in home health, including OASIS documentation, wound care, IV therapy, and management of high-risk medical diagnoses.

Skilled home care experience required
Private duty nursing experience alone will not be accepted

Required Skills & Experience

Home Health experience with OASIS training and hands-on experience
IV therapy skills
Wound care experience
General medical diagnosis management
Post-operative care experience
Experience with high-risk diagnoses including:
- Heart Failure (HF)
- COPD
- Myocardial Infarction (MI)
- Pneumonia
- Stroke

Certifications Required

BCLS Certification

Dress Code

Any solid-color scrubs

Patient Care Responsibilities

Cardiopulmonary assessments
Basic wound care
ABI compression dressing
Wound vac dressing changes
Pleurx/Asept drain care
Foley catheter changes and care
Staple and suture removal
Glucometer usage
Pro-time machine usage
SQ/IM injections
Lab draws and venipuncture
PICC line or Midline care and dressing changes
SQ port access/de-access

Work Environment

Skilled Home Care setting
- Medicare
- Medicare Advantage
- Commercial insurance patients
Average of 6–8 home visits per day

Scheduling Information

Schedules may vary based on HomeCare office needs:

Monday – Friday: 8:00 AM – 5:00 PM
Monday – Friday: 9:00 AM – 6:00 PM
Thursday – Monday shifts
Friday – Tuesday shifts
Alternating weekend rotations

Ace Timesheet documentation system

Additional Requirements

Minimum 1 year of experience required
Valid driver’s license and reliable vehicle required for home visits
COVID vaccination required or approved exemption on file
Flu shot required or signed declination form
Mask wearing mandatory at all times while working
Visits and work hours are self-scheduled based on HomeCare office needs

Pay Rate
$48.06 - $52.94/hour
