Registered Nurse (RN) – Home Health / Skilled Home Care
Shift: Day Shift – 5x8 Hours (8:00 AM – 5:00 PM)
Job Description
We are seeking an experienced Home Health Registered Nurse (RN) for skilled home care visits. Local candidates are accepted at the same rate. The ideal candidate must have strong home health experience, including OASIS documentation, wound care, IV therapy, and management of high-risk medical diagnoses.
Required Skills & Experience
- Skilled Home Care experience required
(Private duty nursing experience alone will not be accepted) - Home Health experience with OASIS training and documentation
- IV therapy skills
- Wound care experience
- Experience with general medical diagnoses
- Experience managing high-risk diagnoses including:
- Heart Failure (HF)
- COPD
- Myocardial Infarction (MI)
- Pneumonia
- Stroke
- Post-operative care experience
Certifications Required
- BCLS Certification
Scrub Color
- Any solid color scrubs
Patient Care Responsibilities
- Cardiopulmonary assessments
- Basic wound care
- ABI compression dressing application
- Wound VAC dressing changes
- PleurX / Asept drain care
- Foley catheter care and changes
- Staple and suture removal
- Glucometer usage
- Pro-time machine usage
- SQ/IM injections
- Venipuncture and lab draws
- PICC line and Midline care/dressing changes
- SQ port access/de-access
Unit / Assignment Information
- Skilled Home Care patients including:
- Medicare
- Medicare Advantage
- Commercial insurance patients
- Home skilled visits: 6–8 visits per day
- Schedule may vary based on HomeCare office needs:
- Monday – Friday: 8:00 AM – 5:00 PM
- Monday – Friday: 9:00 AM – 6:00 PM
- Thursday – Monday or Friday – Tuesday shifts
- Alternating weekends required
- Equipment provided:
- Laptop
- MiFi internet device
- Portable printer
- Documentation systems:
- BayCare Forms
- BayCare Scheduling
- Ace Timesheet documentation system
Additional Requirements
- Minimum 1 year of experience required
- Valid driver’s license and reliable vehicle required for home visits
- COVID-19 vaccination required or approved exemption on file
- Flu vaccination or signed declination required
- Mandatory mask wearing at all times while working
- Visits and working hours will be self-scheduled based on HomeCare office needs
Pay Rate
$48.06 - $52.94/hour
