Registered Nurse – OB Case Manager
Provide specialized case management and coordinated nursing care for pregnant and postpartum patients. The OB Case Manager will assess patient needs, coordinate services, monitor care plans, educate patients, and collaborate with healthcare providers to support safe and positive maternal and fetal outcomes.
Responsibilities
- Perform comprehensive assessments of pregnant and postpartum patients.
- Develop and coordinate individualized care plans.
- Monitor high-risk pregnancies and identify barriers to appropriate care.
- Coordinate medical services, referrals, and follow-up appointments.
- Educate patients on prenatal care, medications, warning signs, and postpartum needs.
- Communicate with physicians, specialists, and interdisciplinary healthcare teams.
- Monitor patient progress and facilitate timely interventions when needed.
- Support continuity of care across healthcare settings.
- Maintain accurate and timely case management documentation.
- Advocate for patients and help connect them with appropriate healthcare resources.
Qualifications
- Current Registered Nurse (RN) license.
- Graduate of an accredited nursing program.
- Previous OB, maternal-child health, case management, or related nursing experience preferred.
- Strong clinical assessment and care coordination skills.
- Excellent communication, organization, and critical-thinking abilities.
- Ability to work independently and effectively with patients and healthcare teams.
Pay Rate
$83.33 - $88.21/hour
