Position Summary:
We are seeking a skilled Physical Therapist to evaluate patients and provide individualized rehabilitation services. The PT will develop treatment plans, provide therapeutic interventions, monitor patient progress, and help patients improve mobility, strength, function, and independence.
Key Responsibilities:
- Perform physical therapy evaluations and functional assessments.
- Develop and implement individualized treatment plans.
- Provide therapeutic exercises, manual therapy, gait training, balance training, and mobility interventions.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on exercises, mobility, safety, and home programs.
- Recommend and train patients in the use of appropriate assistive devices.
- Collaborate with physicians, nurses, PTAs, OTs, SLPs, and other healthcare professionals.
- Participate in care planning and interdisciplinary team meetings.
- Maintain accurate and timely documentation in the EMR.
- Follow infection prevention, HIPAA, patient safety, and organizational policies.
- Promote patient independence, comfort, and quality of life.
Required Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
- Active, unrestricted Physical Therapist license in the state of practice.
- Current BLS certification required.
- Minimum 1 year of recent PT experience preferred.
- Strong knowledge of rehabilitation, therapeutic exercise, mobility, and patient safety.
- Excellent clinical judgment, communication, organization, and patient-care skills.
- Ability to work independently and as part of a multidisciplinary team.
Preferred Qualifications:
- Experience in acute care, inpatient rehabilitation, outpatient, skilled nursing, or home health settings.
- Experience treating orthopedic, neurological, geriatric, post-surgical, or medically complex patients.
- Strong knowledge of gait, balance, strengthening, and functional mobility programs.
- Experience with patient and caregiver education.
- Commitment to compassionate, patient-centered rehabilitation.
Pay Rate
$64.28 - $69.16/hour
