Position Summary:
We are seeking a qualified Physical Therapist or Physical Therapist Assistant to provide rehabilitation services to patients with a variety of physical and functional conditions. The PT will independently evaluate and develop treatment plans, while the PTA will provide treatment under the supervision of a licensed Physical Therapist.
Key Responsibilities:
- Evaluate patients and develop individualized treatment plans within scope of practice.
- Provide therapeutic exercises, gait training, balance activities, manual techniques, and functional mobility training.
- Assist patients with transfers, ambulation, strengthening, and activities designed to improve independence.
- Monitor patient progress and modify treatment interventions as appropriate.
- Educate patients and caregivers on home exercise programs, safety, and injury prevention.
- Recommend and train patients in the use of appropriate assistive devices.
- Maintain accurate and timely treatment documentation in the EMR.
- Communicate patient progress and concerns with the supervising therapist and healthcare team.
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals.
- Follow infection prevention, HIPAA, patient safety, and organizational policies.
Required Qualifications – PT:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
- Active Physical Therapist license in the state of practice.
- Current BLS certification required.
- Strong clinical assessment, treatment planning, and rehabilitation skills.
Required Qualifications – PTA:
- Associate degree from an accredited Physical Therapist Assistant program.
- Active PTA license in the state of practice.
- Current BLS certification required.
- Ability to provide treatment under the supervision of a licensed Physical Therapist.
Preferred Qualifications:
- Minimum 1 year of recent PT/PTA experience preferred.
- Experience in outpatient, acute care, inpatient rehabilitation, SNF, home health, or long-term care settings.
- Experience treating orthopedic, neurological, geriatric, post-surgical, or medically complex patients.
- Strong communication, organization, and time-management skills.
- Experience with EMR systems preferred.
- Commitment to compassionate, patient-centered care.
Pay Rate
$78,956.00 - $110,177.60/year
