Population Health Clinical Pharmacy Specialist
We are seeking an experienced Population Health Clinical Pharmacy Specialist to provide clinical pharmacy services that improve medication outcomes, support chronic disease management, and promote cost-effective patient care.
Key Responsibilities:
- Review medication regimens and identify opportunities to improve therapy.
- Provide medication management and patient education.
- Support chronic disease management and population health initiatives.
- Collaborate with physicians and healthcare teams on medication-related care.
- Monitor medication effectiveness, safety, and adherence.
- Document clinical interventions and patient outcomes.
Qualifications:
- Doctor of Pharmacy (PharmD) or equivalent pharmacy degree.
- Active Pharmacist license as required by state regulations.
- Clinical pharmacy or population health experience preferred.
- Strong knowledge of pharmacotherapy, medication management, and chronic disease care.
- Excellent clinical, analytical, and communication skills.
Pay Rate
$136,000.00 - $200,000.00/year
