Position Title: Clinical Pharmacist
Part-time or Full-time: Full time
Shift times and shift length: M-F between 7:30 and 17:00. Every fourth weekend working half days S/S (approx. 8-12).
Weekend Requirement: Every 4th weekend (half days)
On-Call Requirement: None
Holiday Requirement: as need by client
Floating Requirements: None
Desired Start Date: ASAP
Base Hourly Pay/Salary Range: $120,000-$145,000 (depending on experience)

Job Description/General Duties: This role provides patient-focused pharmaceutical care by reviewing and monitoring medication therapy to ensure safe, effective, and cost-conscious treatment. Responsibilities include medication order verification, dosing adjustments, medication reconciliation, patient education, and participation in multidisciplinary patient care rounds. The pharmacist will also support medication safety and performance improvement initiatives, provide education to staff, and assist with departmental coverage as needed.
Minimum Requirements: Bacherlors degree in Pharmacy | Licensed Pharm Acist - Start of Pharmacy | Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
Preferred Requirements: Pharmacy Doctorate | Completion of an accredited post graduate training program (PGY-1 and/or PGY02) residency program | PALS - Pediatric Advanced Life Support - American Heart Association (AHA)
