Outpatient Hospice/Medical Social Worker
We are seeking a compassionate and experienced Outpatient Hospice/Medical Social Worker to provide psychosocial support and care coordination for patients and families receiving hospice and outpatient healthcare services. The Social Worker will assess patient and family needs, provide counseling and resources, and collaborate with the interdisciplinary care team to support quality of life and continuity of care.
Responsibilities:
- Conduct psychosocial assessments of patients and families to identify emotional, social, financial, and practical needs.
- Provide counseling, emotional support, and crisis intervention to patients and caregivers.
- Assist patients and families with coping with illness, end-of-life care, grief, and loss.
- Develop and implement individualized social work care plans.
- Connect patients and families with appropriate community resources and support services.
- Assist with advance care planning and end-of-life resources as appropriate.
- Coordinate with hospice nurses, physicians, therapists, and other members of the interdisciplinary team.
- Advocate for patients’ needs, preferences, rights, and access to appropriate services.
- Provide bereavement support and referrals when needed.
- Maintain accurate and timely documentation of assessments, interventions, and follow-up.
- Participate in care conferences and interdisciplinary team meetings.
- Follow applicable healthcare, privacy, safety, and professional standards.
Qualifications:
- Master’s degree in Social Work (MSW) preferred; Bachelor’s degree in Social Work (BSW) may be accepted based on position requirements.
- Current social work license or certification as required by the state.
- Previous hospice, medical, healthcare, or outpatient social work experience preferred.
- Knowledge of hospice care, grief support, community resources, and care coordination.
- Strong communication, counseling, advocacy, and organizational skills.
- Ability to work independently and effectively with patients, families, and multidisciplinary teams.
- Compassionate, empathetic, and patient-focused approach.
Pay Rate
$48.27 - $53.15/hour
