Occupational Therapist (OT) – Home Health
We are seeking a skilled and compassionate Occupational Therapist to provide individualized rehabilitation services to patients in their homes. The Occupational Therapist will evaluate functional needs, develop treatment plans, and help patients improve independence with daily activities, mobility, safety, and overall quality of life.
Responsibilities:
- Perform occupational therapy evaluations and functional assessments in patients’ homes.
- Develop individualized treatment plans based on patient needs and goals.
- Provide therapeutic activities to improve strength, coordination, balance, and functional skills.
- Assist patients with activities of daily living and adaptive techniques.
- Assess home environments and recommend appropriate safety modifications.
- Recommend and train patients in the use of adaptive equipment and assistive devices.
- Educate patients and caregivers on home programs, safety, and continued care.
- Monitor patient progress and modify treatment plans as needed.
- Coordinate care with physicians, nurses, PTs, and other healthcare professionals.
- Maintain accurate patient records and treatment documentation.
- Follow infection prevention, patient safety, and professional standards.
Qualifications:
- Current Occupational Therapist license as required.
- Master’s or Doctoral degree in Occupational Therapy from an accredited program.
- Current NBCOT certification preferred or required as applicable.
- Previous home health or clinical experience preferred.
- Knowledge of occupational therapy, rehabilitation, ADLs, and home safety.
- Strong clinical judgment, assessment, and problem-solving skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a home-based caseload.
- Compassionate, dependable, professional, and patient-focused.
Pay Rate
$60.47 - $65.35/hour
