LPN – Rehab | Job Description
- Provide direct nursing care to patients in a rehabilitation setting.
- Monitor patients’ vital signs, symptoms, and overall condition.
- Administer medications and treatments as ordered by the provider.
- Assist patients with activities of daily living (ADLs), mobility, and personal care.
- Monitor and document patients’ progress and response to treatment.
- Collaborate with RNs, physicians, therapists, CNAs, and other healthcare professionals.
- Assist with wound care, treatments, and other nursing procedures within LPN scope of practice.
- Recognize changes in patient condition and report concerns promptly.
- Maintain accurate and timely patient documentation and medical records.
- Follow facility policies, infection-control procedures, and patient-safety standards.
- Provide compassionate support and education to patients and their families.
Pay Rate
$36.82 - $41.70/hour
