Description:
Important: This is an Aya Offer position. The clinician will not connect with the manager before signing. The facility will have 48 hours after signature to review the clinician.
Assignment Details
•Start Date: November 9 or November 16 (ADDITIONAL STARTS WILL NOT BE ACCOMODATED)
•Schedule: 8-hour day shifts
•Schedule Restrictions: 4x10s and 3x12s are not available
•Assignment Setting: Skilled Home Health
•Patient Ratio: 1:1
•Productivity: Approximately 30 points per week; up to 6 patients per day
•Experience Required: At least 1 year of skilled Home Health experience
•First-Time Travelers: Accepted
•Weekend Requirement: Weekend support may be required
•Call Requirement: None
•Required Certification: BLS
•Local Clinicians: Accepted within a 50–75 mile radius at the same rate
•License: Active Oregon license required before the start date; temporary licenses are not accepted
Time Off
•Up to 3 days of RTO may be automatically approved.
•Blackout periods include the weeks of Thanksgiving, Christmas, and New Year’s.
Special Requirements
•Clinicians must have reliable personal transportation.
•The facility makes every effort to group patient visits and minimize driving.
•The service area may occasionally extend up to approximately 60 miles, or 60–90 minutes of total driving per day.
•Home Care Home Base (HCHB) experience is required.

$47.07 - $51.95/hour
