Job Description

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide skilled home health care to geriatric patients. The ideal candidate will have recent home health experience, proficiency with Home Care Home Base (HCHB), and the ability to manage patient visits efficiently while maintaining accurate documentation and timely communication.

Responsibilities

Provide nursing care to geriatric patients in their homes.
Complete patient visits, monitor patient conditions, and report changes as needed.
Maintain accurate and timely clinical documentation using HCHB.
Coordinate patient care with the healthcare team and follow established care plans.
Manage a caseload of approximately 5–7 patients per day.
Meet the required productivity expectation of 30 points per week.
Travel throughout the assigned territory, which may extend up to a 60-mile radius from the office.
Maintain flexibility with patient assignments and occasional weekend work.
Communicate promptly with the clinical team regarding patient updates and care needs.

Requirements

Active LPN license in hand.
At least 1 year of recent Home Health experience.
Previous experience with Home Care Home Base (HCHB) is required.
Current BLS certification.
Ability to meet the weekly productivity requirement of 30 points.
Reliable transportation and flexibility to travel throughout the assigned territory.
Strong documentation, communication, and time-management skills.
First-time travelers are not accepted.

Pay Rate
$43.13 - $48.01/hour
