Position Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide safe, efficient, and patient-centered nursing care across various healthcare settings. The LPN will assist with patient assessments, administer medications and treatments, monitor patient conditions, maintain accurate documentation, and collaborate with Registered Nurses, physicians, and interdisciplinary teams to support quality patient outcomes.
Key Responsibilities:
• Provide direct nursing care to patients under the direction of Registered Nurses (RNs), physicians, and healthcare providers
• Perform patient assessments and monitor changes in patient health status
• Administer medications, treatments, and therapies within the scope of practice and facility guidelines
• Monitor vital signs, symptoms, and patient responses to care interventions
• Assist with wound care, catheter care, respiratory treatments, and other nursing procedures as assigned
• Implement nursing interventions according to individualized patient care plans
• Maintain accurate and timely documentation of patient care, assessments, treatments, and outcomes in electronic medical records (EMR)
• Communicate changes in patient condition and care updates to appropriate healthcare team members
• Assist patients with activities of daily living (ADLs), mobility, comfort, and safety needs
• Provide patient and family education regarding medications, treatments, and care instructions
• Collaborate with RNs, physicians, therapists, and interdisciplinary healthcare teams to coordinate patient care
• Follow infection prevention practices, safety protocols, and regulatory standards
• Support continuity of care and promote a positive patient experience
• Maintain professionalism, confidentiality, and compassionate patient interactions
Required Qualifications:
• Active, unrestricted Licensed Practical Nurse (LPN/LVN) license in the state of practice
• Completion of an accredited Practical Nursing program
• Current BLS certification required
• Minimum 1–2 years of recent LPN experience preferred
• Strong knowledge of nursing principles, patient care procedures, and clinical documentation
• Excellent communication, teamwork, and organizational skills
• Ability to work effectively in a fast-paced healthcare environment
• Strong clinical assessment and critical-thinking skills
• Commitment to providing safe, compassionate, and high-quality patient care
• Experience with electronic medical record (EMR) systems preferred
Pay Rate
$21.71 - $26.58/hour
