Licensed Practical Nurses at VNH are an integral part of the care team, where they assess a patient’s health status and complete nursing interventions in collaboration with the interdisciplinary team members and the patient.
The LPN will:
Provide nursing services to individuals and families in their residence under the supervision of a registered nurse.
Perform prescribed individualized patient care to patients as delegated.
Assist the RN in the preparation, evaluation, and modification of the patient's plan of care as appropriate.
Observe, document, and report to the RN as appropriate, the patient's physical and emotional needs and reactions to care.
Minimum Qualifications:
Must be a Licensed Practical Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
A graduate from an approved school of practical nursing.
2 years experience in home health setting.
