Complete assessments, discharges, advanced directives, and other tasks as assigned on a short term 40 bed sub-acute unit.

Required Skills & Experience:
-One (1) year of LMSW experience.

Preferred Skills & Experience:
-N/A

At least one year of nursing home experience as a social worker a plus.

-Minimum 1 year of LMSW experience and 6 months–1 year of nursing home experience as a social worker.
- Monday–Friday, 9 AM–5 PM (flexible: 8 AM–4 PM or 8:30 AM–4:30 PM); one Saturday shift per month. 35 hours/week.
- Must-Have: Nursing home experience as a social worker. Valid NYSED LMSW license with Primary Source Verification (PSV).
