Job Description: Licensed Master Social Worker (LMSW)

Position Summary

We are seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to provide psychosocial assessments, supportive therapy, and discharge planning services for patients and their families. The ideal candidate will collaborate with interdisciplinary care teams to ensure patients receive appropriate emotional support, care coordination, and community resource referrals throughout their treatment journey.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to evaluate patient needs, emotional well-being, and social support systems.
  • Provide supportive counseling and therapeutic interventions to patients and families.
  • Develop and implement individualized discharge plans to support safe and effective transitions of care.
  • Coordinate referrals to community resources, behavioral health providers, rehabilitation services, and other support agencies as needed.
  • Collaborate with physicians, nurses, case managers, and multidisciplinary teams to ensure continuity of care.
  • Maintain accurate and timely clinical documentation in accordance with organizational and regulatory standards.
  • Advocate for patient needs and assist with crisis intervention when appropriate.
  • Educate patients and families regarding available support services and treatment resources.

Required Qualifications

  • Master’s degree in Social Work (MSW) from an accredited institution.
  • Current LMSW licensure in applicable state.
  • Minimum of one (1) year of LMSW experience.
  • Strong assessment, communication, and interpersonal skills.
  • Ability to work effectively in a fast-paced healthcare or clinical environment.

Pay Rate
$65.67 - $70.55/hour
