Position Summary:
We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in home settings. The RN will assess patient needs, develop and manage care plans, administer treatments, educate patients and families, and collaborate with healthcare providers to promote safe and effective outcomes.
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care, including medication management, wound care, and treatments as ordered
- Monitor patient conditions and report changes to physicians and care teams
- Educate patients and families on disease management, medications, and home care strategies
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals
- Perform accurate documentation of visits, assessments, interventions, and patient progress
- Review and manage patient medications for safety and compliance
- Support patients with chronic conditions, post-acute care needs, and recovery goals
- Participate in quality improvement initiatives and follow home health care standards
Required Qualifications:
- Active Registered Nurse (RN) license in the state of practice
- Associate or Bachelor of Science in Nursing (ASN/BSN) from an accredited program
- Current Basic Life Support (BLS) certification
- Previous home health, hospice, acute care, or community nursing experience preferred
- Strong assessment, clinical judgment, and critical-thinking skills
- Excellent communication and patient education abilities
- Ability to work independently and manage multiple patient care needs
Preferred Qualifications:
- Experience with OASIS assessments preferred
- Home Health nursing experience preferred
- Knowledge of Medicare home health regulations and documentation requirements
- Previous contract or travel nursing experience preferred
- Strong commitment to patient-centered care and quality outcomes
Pay Rate
$52.85 - $57.72/hour
