Home Care RN
We are seeking a skilled Home Care Registered Nurse (RN) to provide compassionate, individualized nursing care to patients in their homes while promoting safety, comfort, and independence.
Key Responsibilities:
Assess patients in the home and develop individualized nursing care plans.
Provide medications, treatments, wound care, and other skilled nursing services as ordered.
Monitor patient conditions and report changes to physicians and the healthcare team.
Educate patients and caregivers on medications, treatments, disease management, and home safety.
Coordinate care with physicians, therapists, and other healthcare professionals.
Maintain accurate clinical documentation and patient records.
Qualifications:
Graduate of an accredited nursing program.
Current RN license in the applicable state.
Current BLS certification.
Previous home health, home care, or clinical nursing experience preferred.
Strong assessment, communication, organization, and patient-care skills.
Pay Rate
$31.13 - $40.47/hour
