Job Title: LPN – Long-Term Care (LTC)
Position Summary
We are seeking a Licensed Practical Nurse (LPN) to provide daily nursing care to residents in a long-term care setting. The LPN will administer medications and treatments, monitor residents, assist with care needs, and support overall patient comfort and safety.
Responsibilities
- Provide direct nursing care to residents.
- Administer medications and treatments as ordered.
- Monitor vital signs and changes in resident condition.
- Provide wound care and other nursing treatments.
- Assist residents with daily care needs as required.
- Document care, medications, and patient information accurately.
- Report changes in condition to the RN or healthcare provider.
- Educate residents and families on care and treatment.
- Follow infection control and patient safety procedures.
- Collaborate with RNs, physicians, and other healthcare professionals.
Qualifications
- Active Licensed Practical Nurse (LPN) license.
- Long-term care, skilled nursing, or geriatric experience preferred.
- Knowledge of basic nursing care and medication administration.
- BLS certification required or preferred based on facility requirements.
- Strong communication and organizational skills.
- Ability to work effectively with residents and healthcare teams.
Pay Rate
$39.00 - $43.88/hour
