Job Description:
Critical Access Hospital with 10 acute/Swing Bed and an attached ER hall with a trauma bay, 2 tx rooms and 1 triage area. (We staff 2 RN's and 1 CNA and 1 Registration Clerk)
Responsible for coordinating and delivering patient care utilizing the nursing process. In the Emergency Room is responsible for identifying patient needs and priorities for care through assessment, planning for and delegating care based on patient needs, executing Provider interventions, and evaluating the patient's response to care and progress towards outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Assess the patient’s physical and psychosocial condition on an ongoing basis throughout the continuum of care.
Develop a plan of patient care reflecting significant nursing diagnosis and interventions to achieve desired patient outcomes, involving the patient/family and healthcare team in this process.
Implement the patient plan of care in collaboration with the healthcare team.
Evaluate patient outcomes, effectiveness of plan of care, and readiness for discharge.
Revise as needed and make referrals when indicated.
Complete required documentation in an accurate and timely manner.
Utilize manual and automated information systems to improve efficiency and effect productivity.
Demonstrate a current working knowledge of equipment, instrumentation, and medications.
Adhere to the standards, policies, procedures, and goals of the Hospital District.
Act as a liaison in coordinating patient care activities with nurses, physicians, and allied health professionals.
Communicate patient’s plan of care and significant changes to all members of the interdisciplinary team and to the patient, family, and significant others.
Participate in and support performance improvement and risk management activities.
Provide education as indicated after assessing the learning needs of the patient, family, staff members and students.
Demonstrate cost effectiveness by using supplies judiciously, anticipating needs for special supplies and equipment, and making recommendations for change.
Complete required continuous training and education, including department specific requirements.
Comply with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infectious body fluids, chemical disinfectants, radiation, asbestos, and other hazardous substances.
Attends meetings and in-service sessions, as directed.
Requires regular attendance and punctuality.
Performs other assigned duties.
Emergency Room Coverage Essential Job Duties and Responsibilities
Initiate assessments based on the patient's presenting problems or needs, the physician's medical diagnosis in a timely manner.
Collects key information from patient, family, laboratory, and other medical reports. Implements T-System and documents assessment, including nursing diagnoses or other clinical judgments.
Completes ongoing assessments as determined by patient condition.
Performs therapeutic nursing interventions as ordered by the Provider consistent with primary and population-specific standards of care and practice.
