Job Title: Home Health RN
Position Summary
We are seeking a Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patient needs, provide treatments, manage care plans, and educate patients and families to support safe recovery and ongoing health.
Responsibilities
- Perform comprehensive nursing assessments in patients' homes.
- Develop and implement individualized plans of care.
- Administer medications, treatments, and IV therapy as ordered.
- Provide wound care and other skilled nursing services.
- Monitor patient progress and report changes in condition.
- Educate patients and families on medications, treatments, and home care.
- Coordinate care with physicians and other healthcare professionals.
- Maintain accurate clinical documentation and patient records.
- Follow infection control and patient safety standards.
- Support patients in managing chronic and ongoing health conditions.
Qualifications
- Active Registered Nurse (RN) license.
- Previous home health experience preferred.
- Knowledge of skilled nursing and home-based patient care.
- Strong assessment, communication, and organizational skills.
- Ability to work independently and manage multiple patient needs.
- Valid driver's license and reliable transportation may be required based on the role.
Pay Rate
$56.66 - $61.53/hour
