Position Summary:
We are seeking a compassionate and experienced Travel Physical Therapist (PT) to provide rehabilitation services for residents in a Senior Living Community. The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, strength, balance, and overall functional independence while enhancing quality of life.
Key Responsibilities:
- Evaluate residents and develop individualized physical therapy treatment plans
- Provide therapeutic exercises, gait training, balance training, manual therapy, and mobility interventions
- Assess strength, range of motion, functional mobility, and fall risk
- Monitor patient progress and modify treatment plans as needed
- Educate residents, families, and caregivers on home exercise programs, fall prevention, and safe mobility techniques
- Recommend and train patients in the use of assistive devices and adaptive equipment
- Document evaluations, treatment sessions, progress notes, and discharge plans in the Electronic Medical Record (EMR)
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate resident care
- Maintain therapy equipment and treatment areas in a clean, safe, and organized condition
- Ensure compliance with infection prevention protocols, HIPAA regulations, patient safety standards, and facility policies
- Participate in quality improvement initiatives and continuing education activities
Required Qualifications:
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited Physical Therapy program
- Active, unrestricted Physical Therapist (PT) license in the state of practice
- Current BLS/CPR certification required
- Minimum 1 year of recent Physical Therapy experience, preferably in senior living, skilled nursing, rehabilitation, or geriatric settings
- Strong knowledge of geriatric rehabilitation, balance training, fall prevention, and mobility restoration
- Excellent communication, organizational, and interpersonal skills
- Experience with Electronic Medical Record (EMR) systems preferred
- Ability to adapt quickly to new clinical environments and travel assignments
Pay Rate
$56.66 - $61.53/hour
