Licensed Practical Nurse (LPN) – Job Description
Position Summary:
The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for administering medications, monitoring patient conditions, documenting care, and ensuring the comfort and well-being of patients.
Key Responsibilities:
- Administer medications and treatments as prescribed.
- Monitor and record vital signs, symptoms, and patient progress.
- Assist with patient assessments and care planning.
- Maintain accurate and timely medical records and documentation.
- Provide wound care and other basic nursing procedures.
- Support patients with activities of daily living (ADLs).
- Communicate effectively with patients, families, and healthcare team members.
- Follow facility policies, infection control standards, and safety regulations.
Qualifications:
- Active LPN license in the applicable state.
- Current BLS/CPR certification.
- Strong communication and organizational skills.
- Ability to work effectively in a fast-paced healthcare environment.
- Previous experience in Long-Term Care (LTC), Skilled Nursing, Rehabilitation, or Acute Care preferred.
