Licensed Practical Nurse (LPN)

Job Summary

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for administering medications, monitoring patient conditions, documenting care, and supporting the overall health and well-being of residents/patients.

Key Responsibilities

  • Provide direct nursing care to residents/patients.

  • Administer medications and treatments as prescribed.

  • Monitor and document patient conditions, vital signs, and responses to treatment.

  • Assist with patient assessments and care planning.

  • Maintain accurate and timely clinical documentation.

  • Communicate changes in patient condition to the healthcare team.

  • Ensure compliance with facility policies, procedures, and regulatory requirements.

  • Assist residents with activities of daily living as needed.

  • Promote patient safety, comfort, and quality care.

Qualifications

  • Active LPN License in good standing.

  • Current BLS/CPR Certification preferred.

  • Previous experience in Long-Term Care (LTC), Skilled Nursing Facility (SNF), Rehabilitation, or Acute Care preferred.

  • Strong clinical, communication, and organizational skills.

  • Ability to work effectively in a team-oriented environment.

Pay Rate
$34.29 - $39.17/hour
