Position Summary:
We are seeking a dependable Transfer Specialist to coordinate and facilitate patient transfers between healthcare facilities and care settings. The Transfer Specialist will manage transfer requests, communicate with clinical teams, coordinate transportation and bed availability, and ensure accurate and timely transfer information.
Key Responsibilities:
- Coordinate patient transfer requests between hospitals, healthcare facilities, and other care settings.
- Receive and review transfer requests and collect required patient information.
- Communicate with physicians, nurses, case managers, bed management teams, and receiving facilities.
- Coordinate bed availability and acceptance with receiving facilities.
- Arrange appropriate transportation based on patient condition and transfer requirements.
- Ensure accurate and timely communication of patient clinical and transfer information.
- Maintain transfer records, documentation, and status updates in appropriate systems.
- Track transfer progress and follow up on delays or changes in patient status.
- Assist with coordinating urgent and emergent transfers as required.
- Protect patient confidentiality and follow HIPAA requirements.
- Identify and resolve transfer-related issues promptly.
- Collaborate with multidisciplinary teams to support safe and efficient patient transitions.
- Follow organizational policies, patient safety standards, and regulatory requirements.
Required Qualifications:
- High school diploma or equivalent required; associate or bachelor’s degree preferred.
- Previous healthcare, patient transfer, care coordination, dispatch, or medical transportation experience preferred.
- Strong communication, organization, and problem-solving skills.
- Ability to manage multiple transfer requests and competing priorities.
- Strong attention to detail and accurate documentation skills.
- Ability to communicate effectively with clinical and administrative teams.
- Proficiency with EMR, scheduling, and communication systems preferred.
- Ability to remain calm and professional in a fast-paced environment.
Preferred Qualifications:
- Previous hospital transfer center or bed management experience.
- Knowledge of hospital workflows, patient placement, and medical transportation.
- Experience coordinating transfers involving critical-care or specialty patients.
- Familiarity with healthcare terminology and clinical documentation.
- Strong customer service and relationship-building skills.
- Ability to work flexible shifts, including weekends and holidays as required.
Pay Rate
$17.02 - $21.90/hour
