Job Summary:
-Provide service to patients who have speech and language problems.
-Participate in the development of speech therapy plans of care for patients.
-Make visits to the patients for initial and ongoing assessments to identify the patient’s level of functioning and ability to communicated, including OASIS Assessments at appropriate time points.
-Communicates with the physician before start of care and as needed.
-Implement the plans of care.
-Provide education and instruction to team members, patients and families.
-Document care and submit paperwork in an accurate and timely manner to update and maintain the patient medical records.

Knowledge, Skills, Abilities

Required Skills & Experience:
-One (1) year of experience (preferably in a Medical setting) is required
-Frequent interactions with providers, colleagues, customers, patients/clients, and visitors require employees to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
-Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
-If driving is required, current driver's license, insured, reliable transportation, and acceptable driving record.
