Social Worker
Position Summary
Provide supportive and coordinated social services to patients and families to address emotional, social, financial, and discharge-related needs. Connect patients with appropriate community resources and healthcare services.
Responsibilities
- Assess patients’ psychosocial needs and develop appropriate support plans.
- Provide counseling, emotional support, and crisis intervention as needed.
- Assist patients and families with discharge planning and care coordination.
- Connect patients with community resources, benefits, housing, transportation, and other support services.
- Advocate for patients and help address barriers to care.
- Collaborate with physicians, nurses, therapists, case managers, and other healthcare professionals.
- Maintain accurate and timely documentation of services provided.
- Follow ethical, legal, confidentiality, and patient-safety standards.
Qualifications
- Bachelor’s or Master’s degree in Social Work from an accredited program.
- Current social work license or certification as required by the applicable state.
- Previous healthcare, hospital, behavioral health, or community social work experience preferred.
- Strong communication, assessment, counseling, and problem-solving skills.
- Ability to work effectively with patients from diverse backgrounds.
Pay Rate
$64.28 - $69.16/hour
