Job Title: Social Worker
Job Summary:
The Social Worker provides psychosocial support, advocacy, and care coordination services to patients and their families. This role focuses on assessing social, emotional, and environmental needs and connecting individuals with appropriate resources to support optimal health outcomes.
Key Responsibilities:

Conduct psychosocial assessments to identify patient and family needs

Develop and implement care plans in collaboration with the interdisciplinary team

Provide counseling, emotional support, and crisis intervention as needed

Coordinate discharge planning and community resource referrals

Advocate for patients’ rights and access to services

Maintain accurate and timely documentation in compliance with facility and regulatory standards

Participate in team meetings, case conferences, and care coordination activities
Qualifications:

Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited program
-
Current state licensure or eligibility for licensure as required
-
Previous experience in a healthcare or clinical setting preferred
-
Strong communication, organizational, and interpersonal skills
-
Ability to work effectively with diverse populations

$18.62 - $23.50/hour
