SNF Occupational Therapist
Position Summary
We are seeking an Occupational Therapist (OT) to provide skilled therapy services to patients in a skilled nursing facility (SNF). The OT will evaluate functional needs, develop individualized treatment plans, and help patients improve independence with daily activities and mobility.
Responsibilities
- Evaluate patients’ functional, cognitive, and physical abilities.
- Develop individualized occupational therapy treatment plans.
- Provide therapeutic activities to improve strength, coordination, balance, and mobility.
- Train patients in activities of daily living (ADLs) and functional tasks.
- Recommend and train patients in the use of adaptive equipment as appropriate.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients, families, and caregivers on home and safety strategies.
- Maintain accurate and timely clinical documentation.
- Collaborate with nursing staff, physicians, PTs, SLPs, and other healthcare professionals.
- Follow patient safety, infection control, and professional standards.
Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Occupational Therapy from an accredited program.
- Current Occupational Therapist license in the applicable state.
- Previous SNF, long-term care, or rehabilitation experience preferred.
- Strong clinical, communication, assessment, and documentation skills.
- Ability to provide safe, patient-centered care.
Pay Rate
$79.52 - $84.40/hour
