JOB DESCRIPTION
RN case manager to provide a range of direct and indirect services for patient/families including, but not limited to, the identification of psychosocial problems and their resolutions. In addition, the RN case manager collaborates with the patient/family/team to effect safe and timely discharge/treatment plans.
Responsibilities:
Complete initial and concurrent insurance reviews
Complete PRI for patients requiring skilled nursing facility placement.
Make psychosocial assessments utilizing all available data, including interviews with patient and family and conferring with interdisciplinary associates.
Ensure continuity of care through development, implementation and validation of discharge/treatment plans, in a timely manner and in collaboration with patient, family and interdisciplinary team.
Function independently but utilize supervision to identify any problems in expediting discharge/treatment plans.
Ensure confidentiality in accordance with hospital and Social Services policies.
Develop ongoing linkages and networking relationships.
Participate in patient focused conferences and in internal and external conferences
Demonstrates leadership skills by utilizing psychosocial expertise to assist with team building, discharge/treatment planning. Conflict resolution and psychosocial education to interdisciplinary staff when appropriate.
Participate in departmental programs, i.e., quality improvement, statistical reporting, continuing education, staff meetings and other related assigned duties.
Demonstrates knowledge of the legal and ethical issues and principles related to end-of-life care, including informed consent, right to refuse treatment, withholding or withdrawing life sustaining treatment, including nutrition and hydration, DNR order, advance directives, determination of decisional capacity and the doctrine of futility.
Demonstrates skill and knowledge related to the identification, reporting and appropriate intervention for victims of individual and family violence, rape, mugging, elder abuse and child abuse/neglect.
Demonstrate appropriate interventions and timely and safe discharge/treatment plans through sound psychosocial assessments. Demonstrate organization of work load with appropriate priorities based on medical plan, patient needs and departmental policy and standards. Seek help/resources to avoid unnecessary delays in service
Demonstrate proactive advocacy/mediating negotiating skills with patients/families/associates/community.
Qualifications:
RN/ BSN Preferred
SCREEN CERTIFICATION preferred
Demonstrates knowledge of the impact of medical, social and environmental factors on patient/family/institutional systems.
Pay Rate
$65.24 - $70.11/hour
