Registered Nurse – Post-Acute Long-Term Care (LTC RN)

We are seeking a compassionate and dedicated Registered Nurse to provide high-quality nursing care to patients in a post-acute long-term care setting. The Post-Acute LTC RN will assess patient needs, coordinate care, administer treatments, and support patients transitioning from acute care while promoting recovery and long-term wellness.

Responsibilities:
• Assess, monitor, and document patient conditions and responses to treatment.
• Administer medications, IV therapies, and prescribed treatments.
• Develop, implement, and evaluate individualized care plans.
• Coordinate care with physicians, therapists, and interdisciplinary healthcare teams.
• Supervise and support LPNs and Nursing Assistants as needed.
• Educate patients and families on treatment plans, medications, and discharge goals.
• Maintain accurate and timely clinical documentation.
• Ensure compliance with infection control, safety standards, and regulatory requirements.

Qualifications:
• Active and unrestricted Registered Nurse (RN) license.
• Graduation from an accredited nursing program.
• BLS certification required.
• Strong clinical assessment, communication, and leadership skills.
• Ability to work effectively in a fast-paced post-acute or long-term care environment.
• Previous post-acute care, long-term care, skilled nursing, or rehabilitation experience preferred.

Pay Rate
$52.85 - $57.72/hour
