Position Summary:

We are seeking a compassionate and experienced Registered Nurse to provide skilled nursing care to residents in a long-term care setting. The RN will oversee resident care, administer medications and treatments, monitor health conditions, coordinate services, and support residents' comfort, safety, and quality of life.

Key Responsibilities:

  • Perform comprehensive nursing assessments and develop individualized care plans.
  • Monitor residents' conditions, vital signs, laboratory results, and changes in health status.
  • Administer medications, IV therapies, and prescribed treatments safely.
  • Provide wound care, catheter care, and other skilled nursing interventions.
  • Manage chronic conditions and identify signs of clinical deterioration.
  • Coordinate care with physicians, LPNs, CNAs, therapists, pharmacists, social workers, and other healthcare professionals.
  • Supervise and support nursing and care staff as appropriate.
  • Participate in admissions, resident assessments, care conferences, and discharge planning.
  • Educate residents and families regarding medications, treatments, health conditions, and care plans.
  • Respond to emergencies and initiate appropriate interventions.
  • Maintain accurate and timely documentation in the EMR.
  • Follow infection prevention, HIPAA, patient safety, and long-term care policies.
  • Promote resident dignity, independence, comfort, and quality of life.

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license in the state of practice.
  • ADN or BSN from an accredited nursing program.
  • Current BLS certification required.
  • Minimum 1 year of recent long-term care, skilled nursing, post-acute, or related nursing experience preferred.
  • Strong knowledge of medication administration, patient assessment, wound care, and chronic disease management.
  • Excellent clinical judgment, communication, leadership, and organizational skills.
  • Ability to manage multiple residents and changing clinical priorities.
  • Experience with EMR systems preferred.

Preferred Qualifications:

  • BSN preferred.
  • Previous long-term care, skilled nursing, rehabilitation, or post-acute experience.
  • Experience caring for geriatric and medically complex residents.
  • Wound care and IV therapy experience preferred.
  • Knowledge of care planning, resident assessments, and interdisciplinary coordination.
  • Strong resident and family education skills.
  • Commitment to compassionate, person-centered care.

Pay Rate
$52.85 - $57.72/hour
