Position Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) – Long-Term Care (LTC) to provide comprehensive nursing care to residents in a long-term care setting. The RN will assess, plan, implement, and evaluate resident care while promoting comfort, dignity, and quality of life through patient-centered care.
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized care plans.
- Provide direct nursing care for residents with chronic illnesses, disabilities, and age-related conditions.
- Administer medications, IV therapies, wound care, and other prescribed treatments.
- Monitor resident conditions and promptly report changes to the healthcare provider.
- Supervise and delegate duties to Licensed Practical Nurses (LPNs) and Nursing Assistants (CNAs/LNAs).
- Coordinate admissions, transfers, discharges, and continuity of care.
- Educate residents and families regarding treatments, medications, and ongoing care plans.
- Maintain accurate documentation in the Electronic Medical Record (EMR).
- Collaborate with physicians, therapists, dietitians, social workers, and other healthcare professionals.
- Ensure compliance with infection prevention protocols, HIPAA regulations, resident rights, and facility policies.
- Participate in quality improvement initiatives and continuing education activities.
Required Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program.
- Current BLS certification required.
- Minimum 1 year of recent Long-Term Care (LTC), Skilled Nursing Facility (SNF), or geriatric nursing experience preferred.
- Strong knowledge of geriatric nursing, medication administration, wound care, and chronic disease management.
- Excellent communication, critical-thinking, organizational, and leadership skills.
- Experience with Electronic Medical Record (EMR) systems preferred.
Pay Rate
$64.28 - $69.16/hour
