RN – Home Health
Provide skilled nursing care to patients in their homes while promoting safety, independence, and quality of life. The Home Health RN will assess patient needs, develop individualized care plans, provide treatments, and coordinate care with patients, families, and healthcare providers.
Responsibilities
- Perform comprehensive in-home patient assessments.
- Develop and implement individualized plans of care.
- Administer medications, IV therapy, wound care, and prescribed treatments.
- Monitor patient conditions and report changes to the appropriate healthcare provider.
- Educate patients and families on disease management, medications, and home care.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Provide patient and caregiver education to promote independence and safety.
- Maintain accurate and timely clinical documentation.
- Follow infection control, safety, and home health standards.
Qualifications
- Current Registered Nurse (RN) license.
- Graduate of an accredited nursing program.
- Previous home health experience preferred.
- Strong assessment, clinical, and care coordination skills.
- Excellent communication and time-management abilities.
- Ability to work independently and travel between patient homes.
Pay Rate
$49.03 - $53.91/hour
