Position Summary:

We are seeking a compassionate Registered Nurse (RN) – Home Care to provide skilled nursing services to patients in their homes. The RN will assess patient needs, develop and implement individualized care plans, administer treatments, educate patients and caregivers, and collaborate with the interdisciplinary healthcare team to promote recovery, independence, and quality of life.


Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized home care plans.
  • Provide skilled nursing care, including medication administration, wound care, IV therapy, catheter care, and other nursing procedures.
  • Monitor patient progress and modify care plans based on changes in condition.
  • Educate patients and caregivers on disease management, medications, home safety, and self-care techniques.
  • Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
  • Perform accurate medication reconciliation and monitor for adverse reactions.
  • Document patient assessments, treatments, and progress in the Electronic Medical Record (EMR).
  • Recognize changes in patient condition and communicate findings promptly to the appropriate healthcare provider.
  • Ensure compliance with infection prevention protocols, HIPAA regulations, and home health standards.
  • Participate in quality improvement initiatives and continuing education activities.

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license in the state of practice.
  • Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program.
  • Current BLS certification required.
  • Minimum 1 year of recent Home Care, Home Health, Medical-Surgical, or Acute Care nursing experience preferred.
  • Strong clinical assessment, care planning, wound care, medication management, and patient education skills.
  • Valid driver's license, reliable transportation, and ability to travel between patient homes as required.
  • Experience with Electronic Medical Record (EMR) systems preferred.
  • Excellent communication, organizational, and time-management skills.

Pay Rate
$31.13 - $40.47/hour
