Registered Nurse (RN) – Case Manager
We are seeking a skilled Registered Nurse (RN) Case Manager to coordinate patient care and ensure patients receive appropriate, timely, and cost-effective healthcare services.
Key Responsibilities:
- Assess patient needs and develop individualized care plans.
- Coordinate care among physicians, nurses, specialists, and other healthcare providers.
- Monitor patient progress and facilitate appropriate follow-up care.
- Educate patients and families regarding treatment plans and available resources.
- Assist with discharge planning, referrals, and transitions of care.
- Maintain accurate clinical documentation and communicate changes in patient status.
Qualifications:
- Graduate of an accredited nursing program.
- Current and active RN license as required by the state.
- Current BLS certification.
- Previous case management, care coordination, or clinical nursing experience preferred.
- Strong communication, organization, assessment, and problem-solving skills.
Pay Rate
$86,000.00 - $117,000.00/hour
