Registered Nurse (RN) – Case Manager
We are seeking an experienced Registered Nurse (RN) Case Manager to coordinate patient care, promote appropriate resource utilization, and support quality outcomes across the healthcare continuum.
Key Responsibilities:
- Assess patient needs and develop individualized care plans.
- Coordinate services among patients, providers, and healthcare teams.
- Monitor patient progress and facilitate appropriate transitions of care.
- Identify barriers to care and connect patients with needed resources.
- Collaborate with physicians, social workers, and other healthcare professionals.
- Maintain accurate case management records and documentation.
Qualifications:
- Graduate of an accredited Nursing program.
- Active RN license as required by state regulations.
- Current BLS certification preferred/required.
- Previous Case Management, Care Coordination, or acute care experience preferred.
- Strong clinical assessment, communication, and organizational skills.
Pay Rate
$61.99 - $66.87/hour
