Position Summary:

We are seeking an experienced RN Case Manager to coordinate comprehensive patient care across the healthcare continuum. The RN Case Manager will assess patient needs, develop care plans, coordinate services, monitor outcomes, and collaborate with patients, families, providers, and multidisciplinary teams to promote quality, cost-effective care.

Key Responsibilities:

  • Conduct comprehensive clinical and psychosocial assessments of patients.
  • Develop, implement, and monitor individualized care plans.
  • Coordinate services among physicians, nurses, therapists, social workers, and other healthcare professionals.
  • Monitor patient progress, treatment outcomes, and barriers to care.
  • Facilitate transitions of care, admissions, transfers, and discharge planning.
  • Identify appropriate healthcare resources and community services.
  • Advocate for patients and ensure care plans reflect patient needs and preferences.
  • Communicate with patients, families, providers, and insurance representatives as appropriate.
  • Monitor utilization of healthcare services and promote cost-effective care.
  • Identify potential gaps in care and develop appropriate interventions.
  • Educate patients and caregivers regarding treatment plans, medications, follow-up care, and available resources.
  • Maintain accurate and timely documentation in the EMR.
  • Participate in interdisciplinary case conferences and quality improvement initiatives.
  • Follow HIPAA, patient safety, regulatory, and organizational policies.

Required Qualifications:

  • Active, unrestricted Registered Nurse (RN) license in the state of practice.
  • ADN or BSN from an accredited nursing program.
  • Current BLS certification required.
  • Minimum 2 years of clinical nursing experience preferred.
  • Previous case management, care coordination, utilization review, or discharge planning experience preferred.
  • Strong knowledge of patient assessment, care coordination, and healthcare resources.
  • Excellent clinical judgment, communication, organization, and problem-solving skills.
  • Ability to manage multiple patients and priorities effectively.
  • Experience with EMR systems required/preferred.

Preferred Qualifications:

  • BSN preferred.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred.
  • Experience in acute care, managed care, home health, or outpatient case management.
  • Knowledge of insurance authorization, utilization management, and discharge planning.
  • Strong understanding of community resources and post-acute care services.
  • Excellent patient advocacy and care coordination skills.
  • Commitment to patient-centered, quality-driven care.

Pay Rate
$49.03 - $53.91/hour
