Registered Nurse (RN) – Case Manager – Hospice
We are seeking a compassionate Hospice RN Case Manager to provide patient-centered care and coordinate services for patients and families receiving hospice care.
Responsibilities:
Assess patients and develop individualized hospice care plans.
Coordinate care with physicians and the interdisciplinary hospice team.
Monitor symptoms and provide appropriate nursing interventions.
Administer medications and treatments as prescribed.
Educate and support patients, families, and caregivers.
Communicate changes in patient condition to the care team.
Maintain accurate clinical documentation.
Provide compassionate comfort and end-of-life care.
Qualifications:
Graduate of an accredited Nursing program.
Active RN license as required by state regulations.
Current BLS certification required.
Previous Hospice or Palliative Care experience preferred.
Strong knowledge of pain management, symptom control, and end-of-life care.
Excellent assessment, communication, and care coordination skills.
Pay Rate
$56.66 - $61.53/hour
